Accepted Baccalaureate Students

The following information should prove helpful to you with your transition from Accepted Student to Hopkins Student! Many of these documents require your follow up, so please make sure to review carefully. Documents you need to read/submit

  • Pre-Entrance Health Form
    Send to: Johns Hopkins University School of Nursing, Office of Admissions & Student Services, 525 North Wolfe Street, Suite 113, Baltimore, MD 21205.
    Note: Any questions about the form should be directed to the University Health Services at 410-955-3250. It is vitally important that we receive your completed Pre-Entrance Health Form by the deadline so that University Health Service can review the Form and give us your health clearance in order to prevent a "hold" from being placed on your registration.
  • Background Check Report with Proof of Criminal History Clearance
    All students must complete a criminal background check through PreCheck prior to orientation. Once your report is complete an email will automatically be sent to Dean Sandra Angell, Associate Dean for Student Affairs, which will show your full history report. You can also view the report for 90 day after it is complete by going to http://mystudentcheck.com/. We highly recommend printing a copy for you records.
  • Photocopy of CPR Card
    Proper CPR certification is required prior to orientation.

    Note: if you complete your CPR certification through an agency other than those listed on the CPR document you will have to provide documentation showing that it covers adult, pediatric and infant CPR and airway obstruction as well as the use of automatic external defibrillator (AED).
  • Official transcripts verifying the completion of courses and/or the awarding of a final degree if you completed courses after your acceptance

    Note: if it is not possible to get an official transcript, a letter on school letterhead validating the completion of all your outstanding coursework and/or indicating the grade or verification of a completed degree will be accepted for enrollment into the program.
  • Dosage Calculation
    Includes information on dosage calculation book and practice problems; this is for your information and does not have to be turned in
  • B.U.R.P.S./Talk Like a Nurse
    Includes terminology that you will use throughout the program
